Elderly Women with ‘Dowager’s Hump’ May Be At Higher Risk of Earlier Death

Hyperkyphosis, or “dowager’s hump” the exaggerated forward curvature of the upper spine seen commonly in elderly women may predict earlier death in women whether or not they have vertebral osteoporosis, UCLA researchers have found.

In a study published in the May 19 issue of Annals of Internal Medicine, researchers found that older white women with both vertebral fractures and the increased spinal curvature that results in the bent-over posture characteristic of hyperkyphosis had an elevated risk for earlier death. The finding was independent of other factors that included age and underlying spinal osteoporosis.

Women who had only hyperkyphosis, without vertebral fractures, did not show an increased risk for premature death.

Hyperkyphosis can be caused by a number of factors besides osteoporosis, including habitual poor posture and degenerative diseases of the muscles and intervertebral discs.

“Just being bent forward may be an important clinical finding that should serve as a trigger to seek medical evaluation for possible spinal osteoporosis, as vertebral fractures more often than not are a silent disease,” said Dr. Deborah Kado, an associate professor of orthopedic surgery and medicine at the David Geffen School of Medicine at UCLA and the study’s primary investigator. “We demonstrated that having this age-related postural change is not a good thing. It could mean you’re likely to die sooner.”

For the study, the researchers reviewed data on 610 women, age 67 to 93, from a cohort of 9,704 participants in the Study of Osteoporotic Fractures. The participants were recruited between 1986 and 1988 in Baltimore, Md.; Minneapolis, Minn.; Portland, Ore.; and Pennsylvania’s Monongahela Valley. Researchers measured spinal curvature with a flexicurve and assessed vertebral fractures from spinal radiographs; they assessed mortality based on follow-ups averaging 13.5 years.

Adjusting for age, as well as osteoporosis-related factors such as low bone density, moderate and severe vertebral fractures, and the number of prevalent vertebral fractures, the researchers found that women with previous vertebral fractures and increasing degrees of spinal curvature were at increased mortality risk from the spinal condition, regardless of age, smoking, spinal bone-mineral density, or the number and severity of their spinal fractures.

These study findings provide evidence that it is not just vertebral fracture alone but the associated increased spinal curvature that may be most predictive of adverse health outcomes. Other studies linking hyperkyphosis to poor health, such as impaired physical function, increased fall risk, fractures and mortality, have been unable to exclude the possibility that vertebral fractures alone were the underlying explanation for the findings.

The researchers note several caveats. This study focused on women, though hyperkyphosis also affects men; measurements for vertebral fractures were based only on height ratios, which could lead to misclassification of other causes of height ratio decreases, such as Scheuermann disease; and the timing of the assessments could have affected the results, though it’s unlikely to have made much difference.

However, this study demonstrates a possible association between hyperkyphosis and increased risk for earlier death independent of the number and severity of vertebral fractures or osteoporosis in older women, the researchers write.

“These results add to the growing literature that suggests that hyperkyphosis is a clinically important finding. Because it is readily observed and is associated with ill health in older persons, hyperkyphosis should be recognized as a geriatric syndrome a ‘multifactorial health condition that occurs when the accumulated effect of impairments in multiple systems renders a person vulnerable to situational challenges.’”

Study co-authors include Arun S. Karlamangla of UCLA; Li-Yung Lui and Steven R. Cummings of the California Pacific Medical Center Research Institute; and Kristine E. Ensrud and Howard A. Fink of the University of Minnesota.

The National Institute of Arthritis and Musculoskeletal and Skin Diseases and the National Institute on Aging funded this study.

After a recent ICPA E-Newsletter went out discussing the Webster technique, I received an e-mail from Ann Sommers, a midwife in Southern California. Her comments were supportive and enthusiastic about her experiences with Doctors of Chiropractic in her 11 years as a midwife and doula. She always has and continues to recommend chiropractic care as part of her clients’ prenatal care.

About the Technique, she had this to say: “I have never known the Webster Technique not to work in supporting a breech baby to turn except for one case where there was placenta previa. I have, however, known of the external version not to work!”

During that time I have encountered many breeches and have only experienced 3 breech deliveries. One woman did not choose to do the Webster Technique and had her baby by c-section. Another was a last minute client whose doctor said her baby was vertex (not) and refused to do an ultrasound to confirm it. She delivered breech in a hospital. Another client of mine had a baby that was vertex until the last week— her baby did a surprise turn.

In following up with a phone conversation, Ann and I were able to discuss the numerous reasons why chiropractic care in pregnancy allows for safer and easier deliveries for both the mother and baby. Her referrals over the years to Doctors of Chiropractic always left her midwifery clients feeling well taken care of and appreciative of her recommendations.

The Doctors of Chiropractic further supported the midwifery philosophy for natural birthing. Those mutual clients approached their births with a greater sense of confidence and trust in their body’s ability to function normally. This on-going support from both practitioners is an incredible asset to the pregnant mother throughout pregnancy and in birth.

Find practitioners who support the natural process of birth. Make regular chiropractic care a part of your prenatal choices as well. Birth is an amazing experience and how you prepare throughout pregnancy along with who you work with will in fact lead to a more fulfilling birth experience.

In this study, they took 363 patients who had some type of female organ problem and they measured their lumbar curve (lower back).  Just as you expected, almost every patient had a loss of the lumbar curve, which means they were Subluxated.  The conclusion of the study was an abnormal change in spinal curvature, which appears to be a significant risk factor in pelvic organ prolapse.

If I cut the nerves to your eye, you would not see anymore.  If I cut the nerves to your lungs, you would not breathe anymore.  What is flowing through the nerves?  Life and Energy!  If you lose the normal spinal curves, nerves get irritated, the less life and energy get tot the body, and the body starts building disease.  You do not get symptoms until enough disease is present,  Why would we ever wait to check our family, when we know they could already be building disease in their body?

Good posture is important for the protection of your spine and nerve system.  When standing upright in  weight bearing posture any amount of lumbar spine (low back) flexion is almost always bad for you..    Forward bending (flexion) puts more stress on the disc and forces the nucleus to the disc backward (that is not good).  Maintaining a normal low back curve (lordosis) during lifting can be very helpful in preventing future injury.

Most doctors are aware of the hazards associated with forward bending (lumbar flexion) motions.  Repeated lumbar bending is a well documented way to cause injury to the intervertebral disc.  Bending combined with twisting has been shown to be particularly dangerous in causing disc injury.   The healthy lumbar lordosis provides protection to the disc.

In Pain from Auto Injury?: How Can Chiropractic Care Help? Dr. Peter Adams at Life Mission Chiropractic in Jacksonville cares for people with automobile injuries.

First of all, DON’T WORRY! You can be helped! Auto injuries are perhaps the most damaging type of trauma to the human spine. It’s estimated that for every 10mph of collision the car absorbs its equivalent to your body being hit by a 40 lb weight. As the car gets hit it moves rapidly away in the direction it’s been hit. At the same time the upper body and neck of the occupants get whipped backwards or sideways very quickly. This is where the term “whiplash” comes from. During a whiplash injury the spinal ligaments, muscles and nerves are strained or torn. The immediate aftermath is inflammation and pain. The long term effects are disc herniation and degeneration if not properly dealt with following the accident. The most reported symptoms of spinal trauma is headaches, neck and low back pain following the accident. Most people who visit the E.R. after minor accidents receive muscle relaxers and pain killers and possibly a prescription for physical therapy.

If you have been injured in an automobile accident you should be aware of some important points for your health and future. A spine that has suffered whiplash is fully injured from the neck all the way through the tailbone to some degree or another. So full spine rehabilitation may be necessary. It is critical to get the spine in motion as soon as possible following an injury. This not only restores normal spinal alignment but also prevents ligaments and muscles that have been torn and stretched from forming scar tissue. If scar tissue is allowed to form the spine will heal permanently in a misaligned position causing progressive arthritis and disc degeneration. It is helpful to receive an MRI study of your spine as well so there is no guess work for your healthcare providers.

Should I receive chiropractic care?
